Position type available is Full-time or Part-time
Work at the direction of the Registered Nurse according to the scheduled duties and complies with Bowes In-Home Care (BIHC)’s policies and procedures and all applicable federal, state, local and Medicare laws and regulations. Responsible for assisting patients in performing daily living activities, such as bathing, dressing, toileting, grooming, ambulation and feeding. Observe changes in resident’s condition, report findings to the registered nurse or licensed therapist and complete all necessary documentation timely and according to BIHC standard. Ensures all service provided is completed according to BIHC standards and expectations.
Requirements
Minimum 1-year experience in Home Health.
Completion of a nursing assistant training program here in Illinois or as prescribed by IDPH.
Copy of Certification or Licensure if applicable.
Copy of CPR certification.
Must complete and pass the competency examination conducted by a registered nurse.
Be able to travel to do patient care.
Accessible 24 hours a day, 7 days a week.
Position Summary
Provide care for the following activities of daily living for patients using appropriate and safe techniques in performing personal care such as bathing, grooming, toileting, feeding, medication reminders, and dressing.
Assist with transfers on patients with sufficient balance to do stand pivot transfers or may use adaptive and safety equipment when available and applicable.
Provide assistance in ambulation and exercise of the patient under the direct supervision of a therapist or by an Agency licensed to provide home health or home nursing services.
May be responsible for emptying an ostomy bag and providing assistance with other patient-directed ostomy care only when there is no need for skilled skincare or for observation or reporting to a nurse.
May assist in and perform mouth care, which may include denture care and basic oral hygiene, including oral suctioning for mouth care.
Observe residents for any changes in condition and report to the supervising Case Manager: registered nurse or licensed therapist.
Maintain personal daily time records according to Agency guidelines.
